Following the firm offer of a place at the BESG, it is necessary
for three health forms to be submitted prior to students starting
school. The first form explains immunization requirements,
and contains a section to be completed by a doctor after examination
of the student (within six months prior to starting at BESG).
The second form is for health information. Both forms should
be completed and returned to the school, marked, "Nurse - Confidential".
Form three is the Medical Proforma form.
