Following the firm offer of a place at the BESG, it is
necessary for three health forms to be submitted prior to
students starting school.
IMPORTANT: MEDICAL FORMS
BESG does not accept children in school without
completed medical forms. Your child will not be able to start
school until these forms are completed.
Form A explains immunization requirements on
page 1, and pages 2 and 3 are the Medical Clearance pages, to be
completed by a doctor after examination of the student, within
six months prior to starting at BESG. Note: All the sections
must be completed and this form must be turned in by the first
day of attendance.
Form B is for health information. Both forms
should be completed and returned to the school, marked, "Nurse -
Confidential".
Form C is the Medical Proforma form.
